It’s no secret that Nevada ranks among the lowest states in the U.S. for access to healthcare. It’s a sad fact that many healthcare organizations are trying to address. To do its part, Prominence Health Plan opened two Prominence Care Centers in 2019 providing more access and shifting the focus to preventative care and solving healthcare complexities with data and technology.

Join us for a discussion and virtual tour of a Prominence Care Center to learn more about how their point of care model addresses the whole patient experience from testing and medication refills to screening tools and analytics. The Prominence team will demonstrate some of the point of care technology in use to screen for common conditions including vascular disease, diabetes, neuropathy, osteoporosis, retinopathy and COPD.

Prominence Health Plan will provide insight into how their Care Management team leverages analytics to tackle the challenges of managing care and offers a more personal approach to wellness and chronic disease management. They will also highlight some of the informatics and population health management tools used to keep patients healthy and their providers up-to-date on the whole health of their patients.
